Job Description
Part-time contract (21 hours/week). Ends September 2027.
Hours of Work: 8:30 a.m. to 4:30 p.m. Mostly weekdays but weekends and evenings may be required. Starting Wage: $24.00/hr.
Reports To: Manager, Clinical Services.
Market and implement First Link™ in North Algoma in collaboration with community partners;
Develop and coordinate support services for people living with Alzheimer’s disease and related dementias (ADRD), and their families;
Provide specialized education and training programs for people with dementia and their families, professional care providers, volunteers, and the general public;
Assist with fundraising and volunteer coordination;
Regular travel throughout North Algoma (Wawa, White River, Dubreuilville, Hornepayne, Chapleau), with occasional travel to Sault Ste. Marie;
Responsibilities:
Administer cognitive assessments using standardized tools.
Competency in writing comprehensive reports to primary care practitioners.
Provide education and support to persons living with dementia through home or office visits and by telephone.
Receive referrals from, and make referrals to primary care practitioners and other community support services. Self-referrals are also accepted.
Advocate on behalf of persons living with dementia and their families for improved services, when necessary and/or requested.
Maintain timely, accurate documentation and confidential records for both clients and their care partners via an electronic documentation system.
Organize and/or facilitate support group meetings for persons living with dementia, care partners, as well as family members.
Develop recreation therapy programs for clients to provide activation and respite as needed.
Education
Conduct First Link Learning Series per prescribed guidelines.
Initiate and maintain public education activities in the community to raise awareness of Alzheimer’s Disease and related dementias.
Design and deliver/coordinate educational programs and training events for a variety of target groups as needed to improve dementia care.
Receive training/certification in U-First and Gentle Persuasive Approaches in Dementia Care as opportunities arise.
Marketing and Promotion
Develop and implement a marketing plan for First Link™ and other Alzheimer Society services.
Establish and foster good working relationships with key partners in the primary care sector (e.g., Family Health Team, physicians, Nurse Practitioners).
Execute the annual Alzheimer Awareness campaign in January, which includes but is not limited to, distribution of promotional materials to local media, responding to requests for interviews, sourcing local personal stories related to ADRD, and meeting with elected officials.
Seek out opportunities to increase public awareness of Alzheimer’s Disease and related dementias and the Alzheimer Society.
Fundraising and Volunteer Coordination
In conjunction with the Coordinator of Volunteers and Special Events, assist with the coordination/implementation of fundraising events.
In conjunction with the Coordinator of Volunteers and Special Events, assist with the recruitment and training of volunteers.
Other
Conduct program evaluations of all support and education initiatives.
Record program statistics and submit to Executive Director monthly.
Represent the Alzheimer Society and clients affected by dementia on community committees as appropriate.
Adhere to program standards and Alzheimer Society policies and procedures.
Monitor program expenditures for travel and educational resources to ensure that they remain within the approved budget.
Attend occasional meetings in Sault Ste. Marie, and participate in Society-sponsored professional development opportunities as identified, which may require travel to Southern Ontario.
Assume responsibility for personal upgrading in knowledge of ADRD, and ongoing professional development.
Assume other duties as may be assigned by the Manager, Clinical Services.
PHYSICAL DEMANDS
No special physical demands are required beyond the performance of general office duties
This position involves extended periods of desk work, including significant periods of sitting at the computer, frequent use of a computer keyboard and screen. There is the opportunity to move away from the work station.
Sensory attention is required for the majority of the work day (looking at computer screen, reading documents, etc.)
Physical ability to fully participate in meetings, presentations and training sessions

Required Skills & Role
Post-secondary education in a health and/or social services field and current registration with a regulated health profession (e.g., Registered Practical Nurse, Registered Nurse, Social Worker).
Minimum 1 to 3 years experience working with persons living with dementia and their families.
Previous experience or knowledge of dementia and its impact on the family unit.
Knowledge of community health care agencies.
Current Standard First Aid/CPR certification.
Excellent written and verbal communication skills.
Excellent presentation skills.
Experience with social media/marketing for fundraising purposes.
Ability to work cooperatively with volunteers, staff, and community partners.
High level of competency with Microsoft Office applications.
Demonstrated organizational skills.
Valid driver’s licence and daily access to a vehicle.
Bilingualism (English/French) is an asset
