Job Description
Michipicoten First Nation is seeking a full-time, permanent Community Health Representative (CHR). Reporting to the Manager, Health & Social Services, the CHR supports the health and wellness of Michipicoten First Nation members through outreach, health promotion, and system navigation. This role connects community members to health and social services and supports safe transitions between care settings, including hospital, primary care, and treatment programs. The CHR collaborates with internal teams and external partners to ensure coordinated, culturally grounded care, reduce service gaps, improve access, and support an integrated, community-driven approach to wellness.

Required Skills & Role
LOCATION: Michipicoten First Nation
TERM: Full-time, Permanent
WAGE RANGE: $23.64 - $26.33/hr
QUALIFICATIONS
Education & Training
· Post-secondary education in health, social services, or a related field
· Community Health Representative certification (or willingness to obtain)
· Algoma Public Health (APH) Food handler training and certification is an asset
Experience
· Two years of experience in a First Nation Health setting
Knowledge, Skills & Abilities
· Knowledge of First Nation culture, traditions, and health issues
· Knowledge of community-based health care
· Proficiency with MS Office, Teams, Outlook and data entry
· Strong communication and interpersonal skills
· Ability to work independently and as part of a team
· Empathy, compassion, and cultural sensitivity
· Strong organizational and time management skills
· Ability to handle confidential information with discretion
Other
· Valid G driver's license and reliable transportation.
· Satisfactory driver’s abstract
· Satisfactory Criminal Records Check
· Standard First Aid & CPR
